Handling Concern - Refer to appropriate department, to IDT itself or deal with immediately.

Handling Concern - Refer to appropriate department, to IDT itself or deal with immediately.
[Resident should be notified, (how concern is handled), by whatever means are necessary, to explain, to his satisfaction, why outcome is as it is, whether it be good, bad or indifferent.]
Role of Resident – Identify NEEDS and PREFERENCES, that are seen as necessary to improve quality of life.
Role of IDT – Help Resident identify N's and P's. [When developing N's and P's, be Realistic, Honest and Viable.
Resident and IDT agree on 4 or 5 N's and P's, that are conveyed to C.N.A., or whomever is deemed necessary. These are part of Treatment Plan. If carried out regularly, we see Residents Quality of Life improve. If NOT, renegotiate N's and P's. Straighten out ground-rules.

Member of IDT meets with Resident 5 - 10 mins per week to find out how N's and P's are going [whether they are carried out or not,] how's he doing, any new biz, etc. For ex. If IDT member is AT, he/she will bring back question to SW, and response will be ASAP.
